On Thursday, May 5th, 2022 from 3:00 pm-5:30 pm we will be hosting physicals for any and all incoming 9th, 10th, 11th, and 12th-grade students who need a physical for the upcoming school year. This includes all athletics, band, color guard, cheer, and any other students who may need a physical. You must be 14 years or older on the physical date to be eligible.
Please sign up for your slot using this link or QR code within the flyer. You must be signed up and have the medical history portion of the physical form completed prior to your arrival. Please see the attached physical form.
Physicals will cost $20 per student and can be paid by cash or check. Please make checks payable to Krum Athletic Training and have the driver's license number on the check.
